If your child missed their Flu vaccination at the school session, we would like to invite you to make an appointment at one of our out of school Catch -Up Clinics at the following venues:
• Reed House 3rd Floor Thursday 28th November 3.00pm - 4.30pm - Annie Reed Road, Beverley, HU17 0LF
• The Octagon Family Hub Wednesday 27th November 3.00pm - 5.00pm - Walker Street, Hull, HU3 2RA
• The Oasis Hub Saturday 7th December 10.00 - 2.30pm - Newland Avenue, Hull, HU5 2NB
• The Octagon Family Hub Wednesday 11th December 3.00pm -5.00pm - Walker Street, Hull, HU3 2RA

If you have already completed a consent form you only need to call or email us for an appointment, if you have NOT already completed a consent form, please complete a form online via www.schoolvaccination.uk before calling or emailing to book.
