Dear Parent/Guardian,
We are attending your Childs school for the FINAL session on Wednesday 26th November to administer any outstanding FLU VACCINATIONS.
If you have already completed a consent form and your child was absent or refused to have the vaccination at the first session you need do nothing more your child will already be included on the catch up list, if you no longer wish for your child to be vaccinated or if the child has had the flu vaccination at the GPs, you must let us know ASAP, please call us on 01482 453690
If you have NOT completed a consent form and wish your child to be included in the second session, please follow the link below or go to www.schoolvaccination.uk and complete a form for your child to receive the vaccination at the catch up session. On the webpage click on E-consent forms at the top of the page to access the online forms.
